Privacy Policy

The DNAFORM's ("us," "we," "our") current privacy and security policies and practices are described in this Privacy Policy. Your use of the Site is subject to this Privacy Policy, which you may at any time print or download (or request a printed copy by contacting us as specified below). We reserve the right to change this Privacy Policy at any time by posting a revised policy at this location.

Our site

The Site logs IP addresses, browser types and other information for system administration purposes and to compile anonymous aggregate statistics. We use this information to measure the number of visitors to each section of our Site and to help make our site more useful. These logs are analyzed to improve the value of the information available on the Site. A user's session may be tracked in some cases, but we do not link sessions or IP addresses to any personally identifiable information.
We use email links, forms and email list subscriptions located on contact pages and other areas of the Site to communicate with users and to allow them to contact us directly with any questions or comments. This information is used to respond directly to questions or comments.

Use of personal information

We may collect, store, and use personally identifiable information (such as name, email address, postal code, and email preferences) when it is voluntarily submitted to us, such as when you register with the Site, e-mail us, or otherwise. ANY COMMUNICATION OR MATERIAL THAT YOU TRANSMIT TO OUR SITE, BY ELECTRONIC MAIL OR OTHERWISE, INCLUDING ANY DATA, QUESTIONS, COMMENTS, SUGGESTIONS OR THE LIKE, IS, AND WILL BE TREATED AS NON-CONFIDENTIAL AND NONPROPRIETARY INFORMATION. YOU SHOULD NOT COMMUNICATE ANY PATIENT HEALTH INFORMATION ("PHI") TO US USING THE WEB SITE.
We may use your personal information to contact you, improve this site, provide you with information that you have requested, or provide you with additional information, which we believe may be of interest to you.
We may also use this information to respond to your inquiries, provide you with technical support and enforce our terms and other policies governing use of the Site. We may combine your information with other information about you that is available to us, including information from other sources.
We will not sell or rent your personal information to any other company or organization. We may share your information with third party service providers who perform billing or other services on our behalf. We may access and/or disclose your personal information to law enforcement officials, regulatory agencies or other third parties as we, in our sole discretion, believe appropriate in connection with an investigation of illegal activity that may expose us to legal liability or costs, to enforce our policies governing the Site and for regulatory compliance. We may also disclose your information in connection with corporate restructuring, merger or consolidation with, or sale of substantially all of our assets to, a third party. We do not guarantee that any entity receiving such information in connection with one of these transactions will abide by this Privacy Statement. By providing us with your information, you agree to the foregoing.
Except as described herein, we will not forward or share your personally identifiable information collected using the Site outside of the purposes of the services that we provide for you, nor do we collect your name and email address for any purposes other than to respond to your query, provide services, or bill for services. Nevertheless, you should be aware that email is not necessarily secure against interception. If your communication contains sensitive or personal information, you may want to send it by postal mail.
If you order services via our Site, we may contact you by e-mail to send you information about your order (e.g., order confirmations, shipment notifications, etc.). We may also contact Site visitors via e-mail regarding their account status, and about changes to any relevant agreements or policies. We sometimes also use this data to establish a profile for your company, or collectively for some or all of our customers, to help us make business. We will also use it for other internal business purposes including aggregating information, data warehousing and mining, cross matching databases and pooling data for effective data management, to understand your needs, improve security of data, for statistical analysis, devising, deploying and measuring marketing campaigns and for such and other reasonable business purposes and operational efficiency improvement. We may remove information when it becomes out of date or inaccurate or when properly requested by you (subject to any legal duty or requirement we may have to preserve it).

Your email address

We may obtain email addresses from our customers to use for certain notifications regarding access to our database systems and contractual or legal matters. We will not use these email addresses for any other purpose without first obtaining your prior written authorization to do so. If you would like us to use email as a primary method of communicating with you, you must let us know in writing about this preference.

This Site collects various information from you in the course of your navigation of the Site, such as the URL of the site you just came from, the browser version you use, your Internet Protocol (IP) address, IP ports, date/time of access, size of data transferred, pages you visit, and other "clickstream" data. We are typically not able to relate such data to you as an individual if you merely browse our Site and do not disclose your identity to us, e.g., in connection with a registration. We also use "cookies" to enhance your Site visit. A "cookie" is information either temporarily or permanently stored in a file on your computer. You can set your internet browser to reject cookies (refer to your internet browser instructions if you wish to do so), but that may limit your use of some convenience features at this Site.

Safeguard Limitations and your responsibility

HOWEVER, YOU ACKNOWLEDGE THAT SECURITY SAFEGUARDS, BY THEIR NATURE, ARE CAPABLE OF CIRCUMVENTION AND WE DO NOT AND CANNOT GUARANTEE THAT YOUR INFORMATION, OR PHI, WILL NOT BE ACCESSED BY UNAUTHORIZED PERSONS CAPABLE OF OVERCOMING SUCH SAFEGUARDS. Furthermore, you are responsible for your compliance with state and federal patient privacy laws. We cannot be responsible for unauthorized access to your information, PHI, or any other incident resulting from your failure to comply with patient privacy laws and regulations in your use and disclosure of patient information, or your failure to implement appropriate technical and administrative safeguards.
